Additional Manufacturer Narrative
H3, h6: given the nature of the alleged incident, the device could not be returned for evaluation. The clinical/medical investigation concluded that following the procedure, the patient experienced mid-flexion instability and reported a lack of confidence in her knee, especially when climbing stairs, due to a sensation of it giving way.Despite reviewing the electronic case report forms (ecrfs), the root cause of the issue could not be identified, and a full medical investigation was not possible due to missing supporting documents.Potential contributing factors such as implant positioning, trauma, component sizing, ligament laxity, or tibial slope changes could not be ruled out.It is unclear how the adverse event was treated, though medication was mentioned. No further treatment details or information about the patient¿s outcome were provided. For the femoral component, insert and baseplate, device batch numbers were not provided, thus, an evaluation of the manufacturing records could not be performed.A review of complaint history for the previous 12 months did not reveal similar events for the listed devices.A review of the instructions for use documents for knee systems revealed that warnings and precautions states that the patient should be warned of surgical risks, and made aware of possible adverse effects. The patient should be warned that the implant can become damaged as a result of strenuous activity or trauma.Besides, possible adverse effects section states that temporary or permanent nerve damage can result in pain or numbness of the affected limb.Periodic, long-term follow-up is recommended to monitor the position and state of the prosthetic components, as well as the condition of the adjoining bone.A review of the risk management files revealed this failure mode was previously identified. The anticipated risk level is still adequate.A historical review concluded that there are no prior actions related to this products and event. At this time, we have no evidence to conclude that the products failed to meet any specifications at the time of manufacture.Factors that could contribute to the reported event include joint laxity or patient condition.Based on this investigation, the need for corrective action is not indicated.Should additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.
