It was reported that, during an internal fixation surgery, during the insertion of distal screw, the screw missed the nail. The screw did not go into the nail and was removed. An additional screw was placed using the perfect circles technique. It seemed like the reason the screw missed the nail was due to the drill guide handle (1) and the 130 rad drill gde drop (1).The procedure was resumed, after a non-significant delay, with a s+n back-up device. No injury was reported as a consequence of this issue.

It was reported that, during an internal fixation surgery, during the insertion of distal screw, the screw missed the nail. The screw did not go into the nail and was removed. An additional screw was addition by performing the perfect circles technique.Upon examining the one (1) drill guide handle and one (1) 130 rad drill gde drop, it seemed that had to be the only reason the screw missed the nail. The procedure was resumed, after a non-significant delay, with a s+n back-up device. No injury was reported as a consequence of this issue.

H2: corrected information in h6 (medical device problem code, component code, type of investigation, investigation findings & conclusions).H3, h6: the associated device was returned and evaluated. The visual inspection revealed that the device presents with heavy wear from use, the hysol barrier is fractured and incomplete with significant material missing, leaving sharp edges; the threads of the interior shaft are deformed; the ball and spring are no longer in their intended position. The coloring of the exterior of the device is faded and worn. There are scratches, scuffs and gouges on all surfaces.Furthermore, the damage found in the visual inspection indicates that the device will not function as intended.A review of the production order did not reveal a manufacturing abnormality that could have caused or contributed to the reported incident.A review of complaint history revealed a similar event for the listed part number over the previous 12 months, but no similar events for the batch based on the historical data, this failure mode will be monitored for future complaints for any necessary corrective actions.A historical review concluded that there are no prior actions related to this product and event. At this time, we have no evidence to conclude that the product failed to meet any specifications at the time of manufacture. This device is a reusable instrument that can be exposed to numerous surgeries.Damage from prolonged use, misuse or rough handling are likely potential factors that could contribute to the reported event. We recommend that all reusable instruments be routinely inspected for wear and damage and replaced as necessary.Based on this investigation, the need for corrective action is not indicated.Should additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.
