Event or Problem Description
It was observed that during the device evaluation, the gastrointestinal videoscope exhibited forceps that couldn't be inserted smoothly.There was no patient involvement.
Additional Manufacturer Narrative
The device was returned to olympus for inspection.Based on the results of the investigation, the following led to the malfunction: a dented channel tube.The probable cause of the damage to the channel tube could be stress such as damage or deterioration of parts, device incorrectly handling during reprocessing and interoperability problem.However, the most probable cause of this complaint is expected or random component failure without any design or manufacturing issue.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
