Clinical investigation: there is a temporal relationship between hd therapy utilizing the custom combiset bloodlines and the patient event of blood loss with subsequent hospitalization and blood transfusion.Although it was reported that a blood leak occurred from the connection of the venous blood line and the needle there is no documentation of any defects or damage noted at the connection site prior to the start of treatment.Additionally, there were no other reported issues leading to the event that would cause the reported disconnection. It was reported that the connection was secure prior to the start of treatment. It was reported that the site at the venous bloodline and needle connection was kept uncovered during the treatment. The 2008t machine operator¿s manual states to ensure visibility of bloodline connection to fistula needles or catheter at all times during the treatment and not to cover the access or bloodlines with a blanket or clothing prior to or during the treatment. The cm stated that the machine did not alarm but that no alarm was expected. It was not reported that the pressures rose or fell outside of the alarm window. It cannot be confirmed what may have caused the custom combi set venous bloodline and the needle to become disconnected leading to the patient blood loss. The manufacturer¿s product evaluation has not been completed.A supplemental mdr will be submitted upon completion of this activity. The adverse event was a direct result of the of the patient¿s venous bloodline becoming disconnected from the needle resulting in the blood loss which required a transfusion. There is no evidence of a product defect or damage occurring with the custom combiset bloodlines at this time.
