It was reported that the patient had an advantage fit blue system implanted during a procedure on (b)(6) 2021.Cystourethroscopy showed no bladder perforation or intravesical abnormalities, and the sling was not visible inside the bladder. She tolerated the procedure well and was transferred to the recovery unit in stable condition. No complications were reported. She was admitted overnight for observation, with plans to remove the foley catheter and vaginal packing the following day before discharge. On (b)(6) 2023, the patient underwent partial excision and revision of a vaginal sling due to mesh erosion and stress urinary incontinence (sui).During the procedure, a 1 cm erosion of the midline sling mesh was identified and treated by transecting the sling, dissecting the arms, and re-tensioning the remaining portion with sutures. The exposed sling arms were excised, and the vaginal incision was closed.Cystourethroscopy revealed a normal bladder with no signs of infection. She tolerated the procedure well and was transferred to recovery in stable condition. On (b)(6) 2025, she underwent excision of eroded vaginal mesh, cystourethroscopy, and urethral bulking with bulkamid. The mesh erosion was found approximately 1 cm proximal to the urethral meatus. The sling was dissected and removed bilaterally to the level of the obturator internus muscle. The periurethral gap was closed for hemostasis, and the vaginal incision was sutured in two layers.Cystourethroscopy showed normal bladder and ureteral function.Bulkamid was injected at four positions around the bladder neck to improve urethral coaptation. She tolerated the procedure well, with an estimated blood loss of 40 cc, and was stable and awake upon transfer to recovery.Additionally, according to her attorney, the patient has since experienced complications including mesh erosion, severe abdominal and flank pain, dyspareunia, infections, worsening urinary symptoms, and loss of enjoyment of life. She also claims to have suffered, or may suffer in the future, physical pain, mental anguish, physical impairment, and medical expenses due to the device implantation.
