Product complaint # (b)(4) investigation summary the charge nurse from (redacted) hospital reached out to us regarding a scheduled revision which will take place on (redacted date). They indicated that the implants would need to be sent off to be investigated as the patient was not satisfied with their previous surgery.Patient had an initial total hip procedure in (redacted city), no other information was provided.Patient then underwent a hip revision surgery in (redacted city), no other information was provided.Currently patient has a pinnacle gription shell (unknown size), with screws (unknown quantity or lengths), a liner (unknown), and a competitor stem and head.(redacted date) surgeon will be removing the stem and the shell; it is not known what implants will be used. The product was returned to j&j medtech orthopaedics for evaluation.Visual analysis of the returned device found that the pinn multihole w/gription 54mm has signs of what appears to be organic material adhered at the outer fixation surface, nothing indicative of a device nonconformance was identified. With the provided evidence and the absence of chronological support for review, there is no clear indication to confirm that there was a weakening of the bond between the implant and the bone interface.A functional test was not performed since it was not applicable to the complaint condition.A dimensional inspection was not performed since it was not applicable to the complaint condition. The overall complaint was not confirmed as the observed condition of the pinn multihole w/gription 54mm would have not contributed to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.Corrected: h3.
