Product complaint # (b)(4).Investigation summary: it was reported that " i am contacting you both on behalf of my sister. You both are listed contacts for depuy/j&j and i would like to report an adverse event. My sister underwent a hip surgery (left, full hip) on the (b)(6) 2024 in the clinic, ireland with dr.Approx 10 days after her surgery she got her first infection.Symptoms were swelling, high temperature, shivers, oozing of the area. The 2nd infection came approx.3 months after surgery the 3rd infection occurred (b)(6) 2025 where she was admitted to hospital (b)(6) the 4th infection occurred (b)(6) and was told she had sepsis / cellulitis 5th infection (b)(6)).With all the issues with infections, she still hears a clicking noise and a pinching feeling coming from the device. She has been literally off work since this surgery.I have attached photos of her hip and records from the hospital regarding the implant.I understand this should have been reported earlier however i did not have all the information / we were unsure what to do.Let me know if there is anything else you need in order to investigate this complaint. Please note i have reported this to the hpra earlier today. Thanks in advance and i look forward to hearing from you soon." the product was not returned to j&j medtech orthopaedics; however, x-rays were provided for review.See attachment product details xrays attached. The x-ray investigation revealed nothing indicative of a device nonconformance.A manufacturing record evaluation was performed for the finished device product code: 101214050 lot number: m20n97, and no non-conformances or manufacturing irregularities were identified. As the device was not returned, an as-received condition could not be assessed, and a dimensional inspection and document/specification review were not completed. The overall complaint was not confirmed as the observed condition of the tri-lock bps sz 5 hi offset would not have contributed to the reported adverse event.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a manufacturing record evaluation was performed for the finished device product code: 101214050 lot number: m20n97, and no non-conformances or manufacturing irregularities were identified.
