Additional Manufacturer Narrative
Additional information has been provided in h.3., h.6., and h.11 the sample was not returned.Photos were provided.Duplicated photos were also submitted via notifeye. One photo displayed the endcap of the carton showing the product information.A second photo displayed the anterior carton surface.A third photo displayed a small plastic bag with the lens case and a company cartridge. The lens was observed in the loading area of the cartridge.Upon magnification of the photo to evaluate, the image was very blurry. The cartridge appears to be posterior surface up.The lens was inside the loading area with a portion of the optic and the trailing haptic extending outside the cartridge loading area. The optic appears misfolded, possibly pressed up.A confirmation of the reported scratch cannot be made from the photo. It cannot be confirmed if viscoelastic was present in the cartridge. No further determinations can be made without a sample.Product history records were reviewed and the documentation indicated the product met release criteria.Qualified associated products were used. The root cause cannot be determined for the reported complaint.Based on the provided photos, the product was inside a plastic bag, the lens appeared potentially misfolded and was partially in the loading area of the cartridge.Due to the image quality it is not possible to confirm the presence of the reported scratch damage or confirm if the lens was pressed up to the interior ceiling, instead of biased down per the instruction for use.The presence of clinical solution on the lens cannot be determined. It is difficult to make a determination of handling / damage without evaluation of the physical sample.A final root cause cannot be determined based on available information. The instruction for use instructs: using holding forceps, grasp the lens by the optic edge and gently place the lens anterior side up into the back of the ovd-filled cartridge. The lens should be inserted until the optic is a little more than half-way inside the cartridge.Use the holding forceps to gently push down on the lens, verifying that the lens is on the bottom surface of the cartridge.Using holding forceps, take the trailing haptic, and gently fold the haptic onto the anterior side of the optic.Slowly grip or push the optic edge to position the lens as far into the cartridge as the forceps will permit, while ensuring the lens remains on the bottom surface of the cartridge and the trailing haptic remains on the optic.Failure to follow these steps may cause the lens to advance incorrectly causing delivery issues and/or damage. The reporter indicated that they did not wish to be contacted.File will be reopened if new information or the sample is received. The manufacturer internal reference number is: 2025-32025 h.10 reflects all related report numbers associated with this product event that have been submitted at this time.
