On may 28, 2025, additional information was received indicating the patient experienced bilateral rupture. On jun 5, 2025, additional information was received indicating the patient also experienced bilateral capsular contracture baker grade iii.The replacement devices were identified as mentor gel breast implants (serial numbers (b)(6).This report is for the left breast prosthesis. On jun 13, 2025, the mentor failure analysis lab received the device for evaluation.Device evaluation summary: the product was returned to mentor for evaluation.Mentor conducted a visual inspection of the returned device visual analysis of the returned device revealed that the siltex uhp 535cc breast implant had parallel lines of shell wear on the anterior aspect. In addition, a tear was found at the edges of the shell wear, measuring approximately 3.0 cm.The evaluation determined that the possible cause of the rupture is consistent with normal wear.Shell wear suggests in-vivo folding or creasing of the device. This may be the result of the following factors: continuous and sustained stresses to the device such as too small breast pocket and folding or wrinkling of the shell in the breast pocket. In some cases, the breast implants may also wear out over time.Regarding the reported capsular contracture condition, there are not enough details to determine the factors that may have caused this condition.Capsular contracture in the patient¿s breast might be the result of the body¿s individual physiological response to the implantation of a foreign object in soft tissue.Further, capsular contracture is a known complication associated with these devices and is referenced in our current product insert data sheet. As part of mentor's quality process, all devices are manufactured, inspected, and released according to approved specifications.Based on the results of this investigation, it was determined that the product met the manufacturing release criteria, and that normal wear may have caused the observed defect.Manufacturer¿s reference number: (b)(4).
