On 28-may-2025, the mentor failure analysis lab received the device for evaluation. On 03-jun-2025, mentor completed the investigation on the suspect medical device.Device evaluation summary: according to the information received, the patient experienced a rupture in the breast implant. The product was returned to mentor for evaluation.Mentor conducted visual inspection of the returned device.Visual analysis of the returned device revealed that the breast implant had an area of siltex cracking on the posterior view.In addition, a tear was noted within the siltex cracking measuring approximately 2.9 cm.The evaluation determined that the possible cause of the rupture is consistent with normal wear.Siltex cracking is defined as a material separation occurring in the siltex layer and can eventually progress through the shell of siltex devices.Siltex cracking is ultimately a result of high stresses. As part of mentor's quality process, all devices are manufactured, inspected, and released according to approved specifications.Based on the results of this investigation, it was determined that the product met the manufacturing release criteria, and that normal wear may have caused the observed defect. No corrective and preventive action (capa) is required now.Additional complaint information monitoring for potential safety signals will be conducted through complaint trending as part of post-market surveillance.Manufacturer¿s reference number: (b)(4).

On 01-jul-2025, mentor received additional information about the event: it was previously reported that the right breast prosthesis rupture was discovered during an implant exchange surgery.New information received states that an ultrasound performed on (b)(6) 2024 diagnosed right breast prosthesis rupture along with silicone-laden lymph nodes in the right axilla. The patient suffered from lateral displacement of the right breast prosthesis and flattening of bilateral inferior poles. This medwatch report is for the patient¿s right breast prosthesis.Refer to manufacturer report number 1645337-2025-08076 for the report for the patient¿s left breast prosthesis.Mentor re-evaluated the suspect medical device.Device evaluation summary: according to the information received, the patient experienced a rupture in the breast implant. In addition, ultrasound results indicated silicone-laden lymph nodes in the right axilla, and there was lateral displacement of the breast implant. The product was returned to mentor for evaluation.Mentor conducted visual inspection of the returned device.Visual analysis of the returned device revealed that the breast implant had an area of siltex cracking on the posterior view.In addition, a tear was noted within the siltex cracking measuring approximately 2.9 cm.The evaluation determined that the possible cause of the rupture is consistent with normal wear.Siltex cracking is defined as a material separation occurring in the siltex layer and can eventually progress through the shell of siltex devices.Siltex cracking is ultimately a result of high stresses.Granulomas are noncancerous nodules that are a common tissue reaction to the presence of a variety of foreign materials, such as silicone.Implant displacement/migration may occur from improper implant sizing and/or placement, i.E., when the implant is too large or the pocket too small or when there has been inadequate preoperative assessment of stresses causing movement of the prosthesis.Implant displacement/migration is a known complication associated with these devices and is referenced in our current product insert data sheet.
