Product complaint # (b)(4).Investigation summary: it was reported that the peripheral locking screw went through baseplate when threading in.The surgeon explanted that same screw and implanted a new one.The product was returned to j&j medtech orthopaedics for evaluation.Visual analysis of the returned device revealed that the locking screw 25 was found slightly cross-threaded from the driving recess.Organic material was found in the same area.No other damage was observed.A manufacturing record evaluation was performed for the finished device product code: 550310025 lot number: 229908, and no non-conformances or manufacturing irregularities were identified. As per inhance¿ surgical technique "peripheral screws must be fully seated within the baseplate manually using the star driver 20 and the driver handle".It is worth noting that "locking screws should not be used under power to fully seat the screw head as this can result in the screw head passing through the baseplate peripheral hole".The observed damaged condition suggests that the locking screw was exposed to excessive forces ultimately contributing to the screw passing through the baseplate as reported.Attempts have been made to obtain more information regarding mating device and technique; however, it has not been made available. Therefore, without concrete evidence or additional information it is not possible to determine a root cause.A functional test was not performed since it was not applicable to the complaint condition.A dimensional inspection was not performed since it was not applicable to the complaint condition. The overall complaint was confirmed as the observed condition of the locking screw 25 would have contributed to the complained issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a manufacturing record evaluation was performed for the finished device product code: 550310025 lot number: 229908, and no non-conformances or manufacturing irregularities were identified.H11 additional narrative: corrected: h3.
