The investigation found the microcatheter returned intact.A segment of blue tubing was also returned, measuring 8.4mm in length with an inner diameter of approximately 0.030in and an outer diameter of approximately 0.038in; furthermore, grooves were observed on the inside. The object was determined to be consistent with a remnant of via 17 jacket tubing. The physical evaluation of the device could not identify the exact conditions or circumstances that led to the introduction of this material, but it is consistent with a deviation in the manufacturing process and has been addressed through the quality process.Clinical risk assessment by manager post-market clinical product safety: based on a clinical and medical review of data and in my professional opinion, no patient harm has been reported as there was no patient contact and/or exposure of the identified material which is consistent with a segment of via 17 jacket tubing material. The therapeutic outcome reported for this patient is the case was reported completed successfully as the physician did not utilize the initial device selected and selected another via device for completion of the case.No immediate and postoperative complications were reported in the data reviewed for this clinical and medical patient risk impact assessment.
