Event or Problem Description
It was reported that, during surgery, while removing a 6.5mm cannulated screw, the tip of one (1) guide pin 230mm x 2.4mm broke off and became lodged in the patient's bone.It was not removed, it was deemed too difficult/time consuming to attempt. The screw removal was done due to surgeon preference and was a planned surgery, not due to any adverse event. The procedure was resumed, after a non-significant delay, with the same device. No injury was reported as a consequence of this issue.

Additional information: h6 and h11.Corrected: h6 (health effect - clinical code and health effect - impact code).H11: the device was not returned for evaluation; therefore, a device analysis could not be performed. The clinical/medical investigation concluded that, in the absence of the requested medical documentation, a thorough medical investigation could not be rendered, nor could the definitive clinical root cause of the reported adverse event be determined.Although a procedural variance vs user error could not be ruled out.The impact on the patient was the planned surgery and the non-significant surgical delay.Device batch number was not provided; thus, an evaluation of the manufacturing records could not be performed.A review of complaint history for the previous 12 months did not reveal similar events for the listed device.A review of instructions for care, maintenance, cleaning and sterilization of smith & nephew orthopedics devices for single use devices revealed that as the devices are sold both nonsterile and sterile and often removed from their original packaging to be placed in a containment device they should be cleaned and/or inspected prior to sterilization. The device should not be reprocessed or reused if comes in contact with blood, tissue or bodily fluids.Reuse may increase the risk of breakage, failure, patient infection, patient injury and revision surgery.A review of the risk management file revealed this failure mode was previously identified. The anticipated risk level is still adequate.A historical review concluded that there are no prior actions related to this product and event. At this time, we have no evidence to conclude that the product failed to meet any specifications at the time of manufacture. This is a single use device, surgical technique or damage from misuse are likely potential factors that could contribute to the reported event.Based on this investigation, the need for corrective action is not indicated.Should the device or additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary.Internal complaint reference: (b)(4).
