It was reported that during an arthroscopic right shoulder stabilization, after 30 seconds of debriding soft tissue, particularly the labrum, metal shavings from the curved incisor blade were observed in the glenohumeral joint space.A straight 4.5 shaver was opened to remove all the shavings from the patient. The surgery was completed using a smith and nephew back-up device instead. There was a surgical delay of less than 30 minutes and no further complications were reported.

H2: corrected information in h6 (investigation findings).H3, h6: the reported device was received for evaluation.A visual inspection revealed the device was not returned with original packaging. The color scheme of the device matches the print.Bio debris is present. No metal shavings or flakes are visible. There are no other visible defects.A functional evaluation was performed on the returned device and found that when connected to a reference mdu, it spins as intended without grinding, seizing, or unexpected noise. No metal flakes were observed during testing. However, a clinical review states that an arthroscopic image was provided that confirms the presence of metal shavings in the glenohumeral joint space.Based on the information provided, a procedural variance could not be ruled out of the reported adverse event.A review of device records showed there were no indications to suggest that the product did not meet manufacturing specification upon release for distribution.A complaint history review found no similar reported events.A risk management review found that the reported failure and/or harm was documented appropriately, and there were no indications to suggest the anticipated risk is not adequate.A definitive root cause for the reported issue could not be determined.Factors that could have contributed to the failure include insufficient irrigation during use or unintended excessive force and side loading of the device.Based on this investigation, the need for corrective action is not indicated.
