Event or Problem Description
|
|
It was reported that during an open sigmoidectomy, using the 31 millimeter extra thick circular stapler, the staple line was incomplete, and the staple did not close properly. After firing, it was observed that only part of the posterior surface was stapled, while the anterior surface remained open.A manual anastomosis was performed using a different manufacturer's suture to replace the staple line, leaving the anastomosis end-to-side.
