Additional information provided in d.9., h.3., h.6.And h.11.The device with the lens was returned loose in the carton. The plunger lock and the lens stop were removed. The plunger was oriented correctly.Viscoelastic was observed in the device. The plunger was advanced to mid-nozzle and has underrode the lens. The lens was still within the lens loading area, positioned on top of the plunger. The trailing portion of the optic was misfolded under, broken, and torn by the advancing plunger underride. The leading haptic was extended on the plunger. The trailing haptic was broken in the gusset area and positioned to the left in the loading area.A device history record review and a non-conformance review of the reported lot number was conducted. The deviation review did not reveal any potential contributing factors to the reported complaint and all corresponding production release specifications defined in the device master record were met.Qualified viscoelastics were indicated. The root cause for the reported complaint could not be determined.A plunger underride was observed and may have been interpreted as the reported complaint. The lens was still inside the loading area on the plunger. The trailing optic portion was misfolded under, broken, and torn. The trailing haptic was also broken. The plunger advanced to mid-nozzle with the lens still in the loading area may indicate the plunger was advanced too rapidly, resulting in the observed plunger underride.Plunger underride may occur: due to rapid advancement faster than the instructions for use (ifu) recommended rate.If the device is overfilled with ophthalmic viscosurgical device (ovd), this can prevent the trailing haptic from being placed properly or move the lens out of position resulting in misfolding. If the operating room temperature is too high (> 23°c / 73° f) lens folding consistency is negatively affected, the lens is more adherent and this may inhibit lens advancement.Any of the above listed causes alone, or in combination, may create the reported event. The manufacturer internal reference number is: (b)(4).
