Event or Problem Description
It was reported, via a healthcare professional, that an enterprise2 4mmx23mm no tip (encr402300/ 9392923) and a prowler select plus 150/5cm (606s255x/ 31492230) were used for an angioplasty procedure.During the procedure, the user reported stent impediment in the microcatheter with loss of cerebral target position and premature detachment in the microcatheter. This event occurred after using a balloon catheter (competitor brand) for balloon dilatation. The stent was impeded in proximal of the microcatheter and could not advance any more. The stent was found detached from the delivery wire in the microcatheter.Doctor removed the microcatheter and stent from the patient and gave up stent implantation and completed the surgery. There was no patient injury report. There was no report of a surgical delay resulting from the event. No further information was made available at the time of this review.

Manufacturer ref#: (b)(4).Updated sections on this medwatch: b4, g3, g6, h2, h3, h6 and h11.Complaint conclusion: it was reported, via a healthcare professional, that an enterprise2 4mmx23mm no tip (encr402300/ 9392923) and a prowler select plus 150/5cm (606s255x/ 31492230) were used for an angioplasty procedure.During the procedure, the user reported stent impediment in the microcatheter with loss of cerebral target position and premature detachment in the microcatheter. There was no report of a surgical delay resulting from the event.(b)(6) the device was returned to j&j medtech for further evaluation.A non-sterile enterprise2 4mmx23mm no tip was received contained in the decontamination pouch.Upon receiving the device, visual inspection was performed, and as described in the event, the stent was detached from the delivery wire, and found inside the luer hub of the concomitant microcatheter. The proximal positioning coil of the delivery wire was found curved. The stent was retrieved from the microcatheter and inspected under magnification. It was observed to be in good condition; there was no structural damage (i.E., no broken struts, no kinks); also, it was noted fully expanded, both ends can be noted as completely flared.Microscopic inspection on the delivery wire revealed a stretched condition on the proximal end of the distal positioning coil.A device history record (dhr) was performed and it indicated this product was final inspection tested at lake region medical and was determined to be acceptable.Although the detached stent prevented functional testing for the impeded condition reported, the customer complaint is confirmed based on the damaged delivery wire.It is possible that in an effort to overcome the impeded condition reported, excessive force was inadvertently applied during the device advancement which ultimately led to damaged on the delivery wire, and the detachment of the stent. It is possible that clinical and procedural factors, including device manipulation and operator's technique, may have contributed to the reported failure. At this time, there is no evidence to support that the issue reported in the complaint is a result of a defect inherently related to the device. As part of the j&j medtech quality process, all devices are manufactured, inspected, and released to approved specifications.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no capa activity is required. It should be noted that product failure could be caused by multiple factors. The instructions for use (ifu) do contain the following recommendations: ¿ do not partially deploy the stent from the introducer.Confirm that the delivery wire does not move relative to the introducer during the removal of the cerenovus enterprise vascular reconstruction device and delivery system from the dispenser hoop.Confirm the tip of the delivery wire is entirely within the introducer.Additional complaint information monitoring for potential safety signals is conducted through complaint trending as part of post-market surveillance. As part of the post market surveillance program, information from this complaint is trended to identify statistical signals for consideration of further correction action.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time.A supplemental report will be submitted if new facts arise which materially alter information submitted in a previous mdr report.
