It was reported that, during a rectosigmoidectomy, there was difficulty closing the circular stapler because tissues were not settling properly in the device. The lead surgeon noticed these issues before firing, removed the stapler from the patient to check for disconnection, and observed the tilted anvil, making the stapler unusable. The device was immediately replaced with another of the same model. No patient complications have been reported as a result of this event.
