Manufacturer ref#:(b)(4) updated sections on this medwatch: b4, g3, g6, h2, h3, h4, h6and h11.Complaint conclusion: as reported by a health care professional, during a mechanical thrombectomy, the delivery wire distal part of an embotrap iii 5 mm x 37 mm (et307537, 24l105av) was kinked before the delivery wire went into the sheath. There was no patient injury reported. No additional information is available. The examination under magnification of the returned embotrap device showed no evidence of damage or deformation on outer cage, inner channel, distal coils and bonds.Minor damage was seen on the proximal coil, with offset coils seen on the device.Kinking of the nitinol shaft was also noted on the returned device. The visual inspection indicates that the returned embotrap device was correctly assembled and manufactured, with all adhesive bonds and joints complete and undamaged. The returned embotrap device was successfully passed through a 0.0195¿ id tube, confirming that the profile conformed to the specification for compatibility with 0.021¿ microcatheters. The ptfe insertion tool was dimensionally inspected and found to be within specification for the inner and outer diameter.Device insertion and delivery assessments were performed using the returned embotrap device and a sample prowler microcatheter. The returned embotrap device was able to be fully inserted into the sample prowler microcatheter and delivered to the distal tip without resistance. In attempt to recreate the reported event, device insertion assessments were also performed using a sample embotrap device and a sample prowler microcatheter. It was believed that using excessive force when inserting the device could result in kinking of the shaft, however this was unable to be recreated. While the complaint event is confirmed, however, the root cause for this complaint could not be determined with the information provided. There is no indication that this complaint was a result of a defect or malfunction of the embotrap device.A device history review (dhr) associated with lot 24l105av presented no issues during the manufacturing or inspection process that can be related to the reported complaint.Multiple attempts to obtain additional information were unsuccessful. If information is provided at a later date, the file will be reopened and processed accordingly. As part of the post market surveillance program, information from this complaint is trended to identify statistical signals for consideration of further correction action.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time.A supplemental report will be submitted if new facts arise which materially alter information submitted in a previous mdr report.
