Investigation evaluation: a product evaluation was performed only by the pictures provided with this report because the product said to be involved was not provided to cook for evaluation. The lot number provided in the labeling photo is not legible. The label in the photo matches the product reported. Our evaluation of the product photo provided confirmed the report. The distal end of the cutting wire has separated from the distal catheter in the picture, and it appears that the cutting wire securing component is present at the distal tip of the cutting wire. However, the photo is not clear enough to determine if all segments of the cutting wire securing component remain attached at the distal tip.A product discrepancy or anomaly that could have contributed to this reported occurrence was not observed. The device history record for the lot number said to be involved was reviewed.A discrepancy or anomaly was not observed with the product that was released for distribution.Investigation conclusion: our evaluation of the photos provided confirmed the report. However, we could not conduct a complete investigation because the product said to be involved was not returned for evaluation.A definitive cause for the reported observation could not be determined.Prior to distribution, all tri-tome pc protector sphincterotomes are subjected to a visual inspection and functional test to ensure device integrity. The functional test includes bowing the sphincterotome to ensure the distal end responds to handle manipulation.A review of the device history record confirmed that the lot said to be involved met all manufacturing requirements prior to shipment.Corrective action: based on the quality engineering risk assessment no corrective action is warranted at this time.A review of the complaint history was conducted.Based on this review, the likelihood of this type of report is rare.Quality assurance will continue to monitor for complaint trends.

Event or Problem Description
During an endoscopic retrograde cholangiopancreatography (ercp) procedure, the physician used a cook tri-tome pc sphincterotome. It was reported that the physician opened the package, then advanced the device through wire guide into endoscope working channel conducted cannulation and charged the power and found that the cutting wire broke. The physician changed to a new one to use.Image of the device showing the cutting wire anchor detached was provided.[subject of report].A section of the device did not remain inside the patient¿s body. The patient did not require any additional procedures due to this occurrence.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence.

Investigation evaluation: the product said to be involved was returned in white plastic bag.Provided with the return was an open box from the lot number provided in the report. The label matches the product returned.A photo provided shows the distal end of the cutting wire has separated from the distal catheter in the picture, and it appears that the cutting wire securing component is present at the distal tip of the cutting wire. Our laboratory evaluation of the product said to be involved confirmed the cutting wire securing component located near the distal end of the sphincterotome has disconnected from the catheter.Due to the catheter and securing component disconnection, the distal end of the cutting wire is no longer connected to the sphincterotome catheter at the distal end.The securing component has a longer section measuring 3 mm and a shorter section measuring 2 mm therefore no part of the device is missing. The longer section of the anchor has bent, but remains connected to the shorter section. There is no evidence of a cautery application on the cutting wire (blackening of the cutting wire was not observed).A clear substance was noted within the clear catheter.A brown substance was observed near the distal tip.The catheter has torn at the most distal band where the anchor exited the catheter.A product discrepancy or anomaly that could have contributed to this reported occurrence was not observed. The device history record for the lot number said to be involved was reviewed.A discrepancy or anomaly was not observed with the product that was released for distribution.Investigation conclusion: a visual inspection of the returned device and photo provided confirmed the cutting wire securing component (anchor) has separated from catheter (w/o detachment).A definitive cause for this observation could not be determined because the actual product handling conditions could not be duplicated in the laboratory setting. This limits our ability to conclusively determine a cause.Separation of the cutting wire securing component and the catheter can occur if the tip of the sphincterotome is over flexed. The instructions for use caution the user with the following comment: ¿do not over flex or bow the tip beyond 90 degrees, as this may damage or cause the cutting wire to break.¿ prior to distribution, all tri-tome pc protector sphincterotomes are subjected to a visual inspection and functional test to ensure device integrity. The functional test includes bowing the sphincterotome to ensure the distal end responds to handle manipulation.A review of the device history record confirmed that the lot said to be involved met all manufacturing requirements prior to shipment.Corrective action based on the quality engineering risk assessment no corrective action is warranted at this time.A review of the complaint history was conducted.Based on this review, the likelihood of this type of report is rare.Quality assurance will continue to monitor for complaint trends.
