Medtronic received information that prior to the implant of this 29 mm transcatheter bioprosthetic valve, while loading the valve, the delivery catheter system (dcs) was over-driven.When moving the dcs, the blue deployment handle cracked/separated.The dcs was not used and the valve was successfully loaded onto a new dcs.No adverse patient effects were reported.Of note, the loader was holding the back of the actuator while turning the deployment handle, as opposed to holding the gray grip.
