Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary : it was reported that 244000600 was broken.221750041 was broken.244000510 was broken.255000100 was broken.255000100 was broken. This event did not happen in surgery. The product was returned to j&j medtech orthopaedics for evaluation.Visual analysis of the returned device does not found signs of breakage at the quickset ace grater handle. However the sample has the white sleeve missing. This condition may occur during the cleaning/sterilization process, particularly when the components are assembled or disassembled. As these procedures are often repeated, there is an increased risk of components going missing. However, without concrete evidence or further information, it is not possible to determine a root cause.A functional test was performed using a lab sample mating device and the assessment does not found nothing indicative of a device nonconformance, the grater handle works as intended.A dimensional inspection was not performed since it was not applicable to the complaint condition. The overall complaint was not confirmed as the observed condition of the quickset ace grater handle would have not contributed to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot : a device history record (dhr) review or manufacturing records evaluation (mre) was not possible because the date code (a0505) provided is not a valid finished goods lot number.H11 additional narrative: added: d9.Corrected: h3.
