Event or Problem Description
|
|
It was reported that one onyx frontier drug eluting stent deformed during positioning and advancement of the stent in vivo. The lesion was severely calcified with 90% stenosis and moderate tortuosity located at the proximal obtuse marginal artery. The device was inspected prior to use and negative prep was performed with no issues noted. The lesion was pre-dilated.The device did not pass through a previously deployed stent.Resistance was encountered during device advancement, but no excessive force was used.It was detailed that the stent failed to cross the lesion. The stent was removed, and the lesion was further pre-dilated with ballooning.Upon attempting to re-insert the device and cross the lesion, a broken stent strut was observed sticking up when attempting to put the stent back on the wire. The procedure was completed using a 2.0x12mm onyx stent. No patient complications have been reported as a result of this event.
