Additional Manufacturer Narrative
H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.The complaint of unable to infuse is confirmed and was determined to be use-related.One 3 fr s/l powerpicc sv was returned for evaluation. An initial visual observation of the returned catheter showed abundant use residues throughout the device. The extension tubing of the catheter was observed to be cloudy with use residues.Blood residues could be seen inside the catheter shaft between the 15 cm and 16 cm depth markers.A functional test of attempting to infuse water into the catheter using a 12 ml syringe revealed the catheter was occluded and the water did not infuse through the device. The catheter was cut proximal to the 15 cm depth marker just proximal to the observed use residues inside the catheter.A functional test of attempting to infuse water into the proximal end of the catheter using a 12 ml syringe after the device was cut proximal to the observed residue occlusion revealed the proximal end of the catheter to be patent to infusion.A functional test of attempting to infuse water into the distal end of the catheter using a 3 fr connector and a 12 ml syringe revealed the catheter was occluded with use residues between the 15 cm and 16 cm depth markers. The catheter was cut to reveal blood and biological residues occluding the catheter between the 15 cm and 16 cm depth markers.Inspection of the returned catheter revealed no potential damage/defect related to the manufacture of the device. This complaint will be recorded for future trending and monitoring purposes.
