Event or Problem Description
Medtronic received information regarding an echelon 10 microcatheter that had resistance. The patient was undergoing a coil embolization procedure to treat a left ophthalmic artery segment unruptured saccular aneurysm. The aneurysm max diameter was 4mm and the neck diameter was 3mm.Blood flow was normal.Vessel tortuosity was moderate. It was reported that the devices were prepared and catheter flushed as indicated in the instructions for use (ifu). There was resistance in the echelon microcatheter during delivery so the devices were removed and replaced to complete the procedure successfully. There was no harm or injury to the patient associated with this event.Additional information was received that clarified that the axium coil had resistance during delivery in the echelon microcatheter.A stryker guidewire also encountered resistance in the catheter.Catheter resistance occurred in the distal section. The coil came out of the introducer sheath smoothly.A continuous saline flush was administered and maintained as indicated in the ifu. There was no kink/damage observed to the catheter or coil after removal.
