H6: the reported device was received for evaluation.A visual inspection observed the returned instrument shows no manufacturing abnormalities.Product was out of the original packaging. No packaging returned. The tip is heavily worn from use.The tip has debris trapped in the tip.A functional evaluation of the device found that the wand produced plasma as expected and had no issues activating coag. The tip is partially clogged from the debris trapped in the tip, pressure could not clear it.No overheating of the device was found.A review of device records showed there were no indications to suggest that the product did not meet manufacturing specifications upon release for distribution.A complaint history review found similar reported events.A risk management review found that the reported failure and/or harm was documented appropriately, and there were no indications to suggest the anticipated risk is not adequate. The root cause could not be determined since the reported malfunction could not be duplicated during the product evaluation process.Factors that can contribute to the reported event include: 1) using prewarmed saline; 2) insufficient suction pressure in order to ensure adequate flow and circulation of saline solution to prevent unnecessary heating which could have caused the temperature to elevate; 3) the user may not have set the controller temperature threshold to the desired value.Based on this investigation, the need for corrective action is not indicated.
