Event or Problem Description
It was reported, split in picc tubing.Additional information provided: the crack was discovered by the oncology nurses last week.They were unable to flush it, got an x-ray and could see the crack in patient's arm and ended up pulling it.There was no patient harm done.We reinserted another picc into patient on july 2nd.
