As reported by the user facility: event 1: we had 2 incidents of filter extension set 0.2 micron filter leaking with chemo.No injury reported.

This report has been identified as b.Braun medical internal report number (b)(4).One (1) unused sample in unopen packaging was provided for evaluation.The returned sample underwent visual and physical inspection, with no signs of damage observed.A subsequent functional leakage test was performed, and no leakage was detected.Based on the investigation finding, the sample was within internal specification; therefore, the reported defect was not confirmed.A review of the discrepancy management system (dsms) database was performed for the reported lot number and no abnormalities or non-conformances were noted during the in process or final product inspection.We will maintain this report for further references and continue to monitor other reports for similar occurrences.If any additional pertinent information becomes available, a follow up will be submitted.
