Batch review performed on 11-07-2025 lot 2155927: (b)(4) items manufactured and released on 19-11-2021.No anomalies found related to the problem. To date, no similar events have been reported on the same lot during the period of review.Preliminary investigation performed by r&d project manager: the screwdriver shows a broken hexagonal tip.The failure occurred for excessive torque on the tip since it is visible a twisting of the base portion of the hexagonal tip and is visible typical torsion surface fracture. The main causes of the failure could be: the screwdriver tip was not completely inserted inside the screw hexagonal hole so the tip broke due to unexpected loading condition. The torque limiter connected to the scredriver was not able to limit the maximum torque at 25nm for unknown reasons. An analysis on the broken instrument will be performed.

H6 annex c and d updated.External supplier analysis: the device concerned was manufactured and delivered in june 2019 and has since been in continuous use.Considering its age and the observed condition, including traces of impact and wear, the failure can reasonably be attributed to long-term use and environmental exposure. The internal inspection revealed that corrosion had spread throughout the mechanism, originating from the detachment of the peek bearing that compromised the sealing integrity. This seal failure likely allowed humidity ingress over time, ultimately leading to the malfunction of the torque mechanism. No similar issues have been reported for this product family, which supports the assessment of an isolated case related to ageing and extended service life. Conclusion: although it cannot be confirmed, as the device could not be recovered, from the event description and the details received is possible that the tip of the screwdriver broke due to excessive torque.Upon inspection, the torque limiting mechanism of the wrench reported to have been utilized, was found to be damaged likely due to progressive wear over its useful life.Medacta applies a 100% functional verification prior to release, and the investigation of complaint history data concludes this to be an isolated case with no trend associated or prior similar events. The device history record reviews do not indicate any potentially related manufacturing cause.

During final tightening of the definitive screw the tip of the short screwdriver 5mm broke inside the screw; it could not be recovered. The screw seemed to be completely tightened and the construct seemed stable therefore the broken tip was left in place and the patient was closed. The tip has been removed along with the entirety of the construct about one week later during a revision surgery aimed at correcting malpositioning (falsa via) of the distal stem ((b)(4)).It was reported by the notifier that the torque limiter was used during the first surgery and that it reached its target torque.
