As reported, approximately 9 years post the initial right total knee arthroplasty, the surgeon performed a synovectomy surrounding the affected knee.Mild osteolysis was present. The femoral and tibial components were well fixed. The components were removed and replaced by a competitor knee. There was no reported breakage of a device or surgical delay/prolongation.The patient was last known to be in stable condition following the event.

This follow-up report is being submitted to relay additional and/or corrected information. The following sections were updated: h6, h11 the following sections were corrected: h6 no device was returned for evaluation; photographs of the device were provided; however, the reported event was unable to be confirmed. The review identified that the explanted femoral component and tibial tray look to have residual biological material and/or cement on them, but otherwise appear unremarkable. It was also stated in the event report that the "femoral and tibial components were well fixed." there appears to be potential signs of wear starting on the patella component, however, this is unable to be confirmed based on the provided images and information. The explanted tibial insert appears to have regions of wear damage on the articular surface, including pitting, delamination, and third body wear. The damage on the medial side appears to be toward the anterior surface, while the damage on the lateral side appears more posterior. These characteristics may indicated relative external rotation of the femoral component through the patient's range of motion. The spine of the tibial insert has been sheared off. This appears to have been due to the explantation process and is not otherwise indicated in the event report.Operative notes and/or medical records were not provided for review of usage/ technique.A review of complaint history determined that no further action is required as no trends were identified.A definitive root cause was unable to be determined; however, may have resulted from tibial insert prosthesis wear.Possible causes of the observed polyethylene wear include third body wear, patient-related factors, orientation of the components, inclusion of the polyethylene in the packaging recall and/or any combination of these possibilities. If any further information is obtained that would change or alter any information provided, a supplemental report will be filed accordingly.
