Event or Problem Description
It was reported, "experienced doctor with the avanos introducer kit had problems in the last intervention with the pexias.Gastrostomy tube placed with the introducer kit on (b)(6).On (b)(6), the pexias were not in place but the tube was correctly placed without inflammation signs in the insertion place.¿ no injury or medical interventions reported.
