Medtronic received a report that the right posterior communicating artery aneurysm underwent stent-assisted embolization. The second coil was filled with apb-1-3-hx-es.During the adjustment process, it was found that it was difficult to push and pull out, so the microcatheter was withdrawn from the body together with the spring coil. After detachment, the spring coil was in a stretched state and the operation was ended. The coil was stuck in the catheter. The catheter was not damaged. The coil was damaged. No patient symptoms or further complications were reported as a result of this event. The device and any accessories were prepared as indicated in the ifu.The patient was undergoing surgery for treatment of a saccular, unruptured right posterior communicating artery aneurysm with a max diameter of 2.3 mm and a 1.6 mm neck diameter. It was noted the patient's blood flow was normal and vessel tortuosity was normal.

Additional information was received. The cause of the resistance and stretching could not be determined. The coil exited the introducer sheath smoothly, and a continuous saline flush was administered and maintained in accordance with the ifu.It was reported that after detachment, the spring coil was in a stretched state, though it could not be determined whether premature detachment had occurred. The procedure was not completed as planned; the second coil could not be implanted, so it was withdrawn and the operation was ended.

Product analysis as found condition (condition of returned device): an axium prime coil was returned for analysis within a shipping box; within sealed biohazard pouch and within an opened axium prime coil inner pouch. The echelon-10 micro catheter used during the event was not returned. The axium prime coil was returned without the introducer sheath.Visual inspection/damage location details: the axium prime coil pushwire was found to be broken at break indicator and bent at ~81.1cm from proximal end.The axium prime coil was found to be stretched and damaged within introducer sheath. No other anomalies were observed.Testing/analysis (including sem reports): n/a conclusion: based on the device analysis and reported information, the customer¿s report of ¿coil resistance/stuck in catheter¿ could not be confirmed as the axium implant coil could not be used for resistance testing with an in-house micro catheter due to its damaged condition.Possible contributing factors to ¿coil resistance/stuck in catheter¿ include the use of an incompatible device for delivery, severely tortuous patient anatomy, failure to hydrate the axium implant coil prior to use, and lack of continuous flush during delivery.Based on the device analysis and reported information, the customer¿s report of ¿coil stretch¿ was confirmed. There was no reported issue pushing the axium prime coil out from within the introducer sheath. Therefore, it is possible the axium coil became damaged upon removal from within the introducer sheath; however, the root cause could not be determined. The echelon-10 micro catheter has a labeled id (inner diameter) of 0.017" with two separate marker bands.Per axium prime coil instructions for use (ifu): ¿axium prime coils should be delivered through a microcatheter with a minimum inside diameter of 0.0165¿-0.017¿ with two marker bands. Therefore, the echelon-10 micro catheter was found to be compatible for use with the axium implant coil and can be ruled out as a potential cause.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
