Intuitive surgical, inc.(isi) did receive a da vinci product to perform failure analysis. The medium-large clip applier instrument was found to have the pivoting clamp dislodged from the sector gear.As a result the instrument exhibited imprecise motion.More specifically, the instrument grips failed to open. The root cause of this failure is attributed to user.Additional findings: the instrument was installed on an in-house system and driven. The instrument exhibited imprecise motion. The grip tips moved in the direction commanded, however the grips failed to open. The imprecise motion was a result of the pivoting clamp becoming dislodged from the sector gear. The root cause of this failure is attributed to user. The instrument was found to have a bent drive rod.The drive rod is secured within the pivoting clamp. The root cause of this failure is attributed to user. The instrument will be transferred to engineering for further review. The root cause of this failure is attributed to user. The complaint was confirmed based on the failure analysis. The probable root cause is attributed to the user.

Intuitive surgical, inc.(isi) advanced failure analysis (afa) team re-evaluated the 6mm medium-large clip applier instrument where, initial results were confirmed. The instrument exhibited a dislodged pivot clamp, and the drive rod exhibited a bend. The probable root cause of the dislodged pivoting clamp is attributed to damage during use, which may result from an accidental drop of the instrument or inadvertent collisions with other instruments or hard surfaces.A dislodged pivot clamp can result in the instrument's jaws being unable to be opened or closed as the pivot clamp needs to be seated in the roll sector gear in order for the grips to be actuated.Correction : h8.Was updated to initial use of device.
