Product complaint # (b)(4) investigation summary according to the information received, ¿n/a - instruments found within broken instruments tub at hospital¿.The product returned to medtech orthopaedics for evaluation. The medtech orthopaedics team conducted a visual inspection of the returned device.Visual analysis revealed that ab suction cup inserter exhibits an overall worn appearance consistent with normal and constant use for a long period of time.Additionally, the silicone plunger was not returned for evaluation. However, no signs of breakage was observed. The observed condition of the missing sub-component may occur during the cleaning or sterilization process, particularly when the instrument is being assembled or disassembled. As these procedures are often repeated, there is an increased risk of components going missing.Additionally, the silicone plunger is a removable and replaceable part of the ab suction cup inserter instrument, which further contributes to the possibility of it going missing. However, without concrete evidence or further information, it is not possible to determine a root cause.A dimensional inspection and functional testing were not performed as it is not applicable to the complaint condition. The overall complaint was not confirmed as the observed condition of the ab suction cup inserter would not contribute to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot a device history record (dhr) review or manufacturing records evaluation (mre) was not possible because the date code (ht0807) provided is not a valid finished goods lot# h11 additional narrative: added: h4 corrected: h3.
