Event or Problem Description
It was reported to boston scientific corporation that a wallflex biliary stent was to be implanted in the common bile duct to treat a stricture during an endoscopic retrograde cholangiopancreatography (ercp) procedure performed on (b)(6) 2025.During the procedure, the inner sheath of the delivery system fractured, leaving approximately 10 - 15 cm of foreign material in the ampulla. The fragment was successfully retrieved using rat tooth forceps and the stent was successfully implanted. There were no patient complications reported as a result of this event.

Block h6: imdrf device code a0501 captures the reportable event of inner sheath detached.Imdrf impact code f2301 captures the reportable event of rat tooth forceps used to retrieve the device fragments.Block b5 has been corrected based on the additional information that was received on july 07, 2025.Block h11: the wallflex biliary stent was successfully implanted, and the delivery system has not been returned. Therefore, product analysis could not be performed. However, the documentation provided includes photographs of the stent.A media analysis was performed where it can be observed the device with evidence of inner sheath detached.Media analysis confirmed the reported event of inner sheath/shaft detachment of device or component. However, without proper evaluation of the device it remains unknown the most probable causes that contributed to the event. There is not enough evidence to determine if the reported event was related to the physician's technique during the procedure, due to the anatomical conditions or related to a device malfunction.Taking all available information into consideration, the most probable cause of the reported event is cause not established.
