The device was returned to olympus for inspection when the reportable malfunction was observed.Based on the results of the investigation, the event appears to be stress related, however, a definitive root cause could not be established.It is likely the following led to the malfunction: problems caused by either excessive or inadequate physical force exerted on it by another object resulting in problems e.G.Wear, bending, deformation, fracture, fatigue.Olympus will continue to monitor field performance for this device.

It was observed during olympus' device inspection that the bronchofibervideoscope exhibited a broken piece of the probe.There were no reports of patient involvement.
