It was reported that a patient underwent an atrial flutter (afl) ablation procedure with an octaray mapping catheter and after working for most of the case, toward end of the case, the catheter would not flush. They tried flushing with a syringe, but the issue persisted. The catheter was replaced, and the problem was resolved. The case continued. No adverse patient consequence was reported.Additional information was received which indicated that they were able to map with the octaray.Later in the case, this happened when putting the catheter back in to map.The octaray was not hooked up to a pump.It was hooked up to a flush bag with an intraflow connection of 3ml/min.There was no error on the pump. The correct catheter settings were selected on the generator. There were no issues with flow rate change at the start of ablation.
