Additional information provided in d.9., d.10, h.3., h.6.And h.11.Only the lens was returned. The lens was returned positioned incorrectly (posterior surface up) in the lens case.Viscoelastic was dried on the lens. One haptic was bent in the gusset area and broken in the distal area. The broken haptic portion was returned adhered to the large post of the lid.The other haptic was pulled from the haptic insertion area of the optic. This haptic was bent in the gusset area and broken in the distal area. This broken haptic portion was not returned. The optic was torn, split, and cracked through this haptic insertion area of the optic. The optic damage was observed on the anterior surface and directly opposite on the posterior.A device history record review and a non-conformance review of the reported lot number was conducted. No deviations were identified and all corresponding production release specifications defined in the device master record were met.The associated cartridge and handpiece information was not provided. The company model is only qualified for use in the company cartridges with a qualified handpiece. It is unknown if a qualified cartridge and handpiece were used. Two viscoelastics were indicated. One of the viscoelastics is not qualified for use with this lens when used with any qualified associated cartridge and handpiece combinations.Only the lens was returned.Bent and broken haptic damage was observed. The root cause for the reported complaint may be related to a failure to follow the (instructions for use) ifu.A non-qualified viscoelastic was indicated. The ifu instructs: company foldable iols are qualified for use with an company qualified delivery system (handpiece and cartridge) and ophthalmic viscosurgical device (ovd) combination. The ifu instructs that an company qualified delivery system and viscoelastic combination should be used. The use of an unqualified combination may cause damage to the lens and potential complications during the implantation process. The ifu instructs: use holding forceps to grasp the lens by the optic edge and gently place the lens anterior side up into the back of the ovd-filled cartridge. The lens should be inserted until it is centered with or slightly past the outline etched into the top of the cartridge.Lenses with a 6.5 mm optic diameter may need to be pre-folded for easy entry into the back of the cartridge.Lens may be pre-folded by gently applying pressure to the edge of the lens while holding the central optic with forceps. The trailing haptic will extend from the proximal end of the cartridge.Position the trailing haptic to the left of the haptic post as shown in the detailed view. This will allow the plunger to go past the haptic during the initial advancement of the plunger into the cartridge.Verify the lens is positioned on the bottom surface of the cartridge. The haptic should be maintained to the left of the post when the lens is loaded correctly.Accurate positioning of the lens will decrease the potential for optic and haptic damage.Haptic manufacturing is a validated process with multiple inspections to verify the quality of the produced haptic material. These inspections include a verification of the dimensional accuracy, tensile strength, flexibility and cosmetic appearance. The inspections ensure that the material meets all required specifications before it is allocated for use.The company 27.5 diopter lens for this file was indicated to be the back up lens for the initial lens, company 29.0 diopter lens.Specific clarification was not provided for the selection of a back up lens with a difference of 1.5 diopters. The patient was left aphakic. The manufacturer internal reference number is:(b)(4).H.10 reflects all related report numbers associated with this product event that have been submitted at this time.
