During an endoscopic retrograde cholangiopancreatography (ercp) procedure, the physician used a cook snaploc wire guide locking device. It was reported that the locking device broke into pieces whilst the endoscopist was at crucial position with wires in the pancreatic duct. The wires then had to be removed to replace the device.Whilst the biliary procedure was successful, it was not possible to re-cannulate the pancreatic duct, which led to a significant pancreatitis risk for the patient.[loss of wire guide access - subject of this report].A section of the device did not remain inside the patient¿s body. The patient did not require any additional procedures due to this occurrence.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence.
