Event or Problem Description
It was reported the subject device had sparks from resectoscope.It occurred during a therapeutic tur-p (transurethral resection of prostate) procedure.The procedure was completed using the same device.There were no reports of patient harm.

This supplemental report is being submitted to provide the results of the final investigation.Updated fields: d8, d9, h2, h3, h6, h11 the device was returned to olympus for inspection, and the reported failure was confirmed.In addition, the following reportable malfunctions were identified during the device evaluation: cable was broken in the area of the straight plug and that the plug itself had been detached and the core wire at the broken section was found to be melted and burned.Based on the results of the investigation, a root cause could not be identified.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
