Event or Problem Description
As reported by the user facility: event 2: we had 2 iv tubing breaks while using saline flushes on them causing patient's blood to leak out of iv.

This report has been identified as b.Braun medical internal report number (b)(4).A review of the discrepancy management system (dsms) database was performed for the reported lot number and no abnormalities or non-conformances were noted during the in process or final product inspection.One photograph was provided for investigation.Upon evaluation of the image, it was determined that a proper evaluation could not be performed from the photo of the back of the blister packaging provided.The reported issue was not confirmed.We will maintain this report for further references and continue to monitor other reports for similar occurrences.If any additional pertinent information becomes available, a follow up will be submitted.
