Event or Problem Description
Medtronic received a report that a dense mesh stent was implanted for the right v4 aneurysm. The diameter of the distal anchor point was 2.9 mm, and the diameter of the vessel at the proximal landing point was 3.1 mm.Due to limited models, a ped2-325-25 dense mesh stent was selected for implantation. The access was to use a 6f delivery catheter and 6f115navien.Through the guide wire, the phenom27 stent catheter was advanced to the p1 segment. The guide wire was withdrawn and high-pressure infusion was performed for hydration. After the delivery sheath was pressed against the phenom27 hub, the stent was delivered to the p1 segment. After the stent catheter was came out, the tip end was opened. The stent tip opened abnormally. After locking the y valve, it was pulled back to the basilar artery and the stent tip was deployed again. The stent opening was observed under fluoroscopy and still abnormal. The y valve was locked and the push-pull operation was repeated, but the stent adherence to the wall did not improve.Considering that the stent itself may have product quality problems, the surgeon considered replacing the stent, and then adopted conventional atalas-assisted embolization, so the surgery ended and the surgery was successfully completed. There was failure to open in the distal section. The pipeline was not positioned in a bend.More than 50% of the pipeline had been deployed when it failed to open. There were no additional steps or other devices required to open the pipeline. No patient symptoms or further complications were reported as a result of this event. The pipeline was used for an approved indication. The device and any accessories were prepared as indicated in the ifu.The patient was undergoing surgery for treatment of a saccular, unruptured right v4 aneurysm with a max diameter of 3.1 mm and a 3.4mm neck diameter. The distal landing zone was 2.9mm and the proximal landing zone was 3.1mm.It was noted the patient's blood flow was xxx and vessel tortuosity was moderate. The angiographic result post procedure was parent artery and distal vascular patency.

Additional Manufacturer Narrative
H3.Product analysis:¿ equipment used: video inspection system (m-85519), camera (panasonic lumix dmc-zs5); ruler (m-83361) ¿ as found condition: the pipeline flex shield was returned inside a biohazard bag and a shipping box.¿ visual inspection/damage location details: the distal and proximal dps restraints were found to be intact. The dps sleeves were found intact with no signs of damage. The distal hypotube and ptfe shrink tubing were found to be intact with no signs of elongation. The braid's distal end was not opened and frayed. The proximal end was found open and frayed. No bends were found with the pushwire. No defects were found with the tip coil, distal marker, re-sheathing marker, re-sheathing pad, or the proximal bumper. No other anomalies were observed.¿ conclusion: based on the device analysis and reported information, the customer report of ¿failure/incomplete open distal¿ was not confirmed as the braid's distal and proximal ends were found opened and frayed. The cause of the ¿failure/incomplete open¿ could not be determined.Possible causes of ¿failure/incomplete open¿ include vessel tortuosity, damaged braid, and deployment of the braid in the vessel bend. However, the customer indicated that vessel tortuosity was moderate, and the braid was not positioned in the vessel bend, which eliminates these factors out as potential causes. In this event, the damaged braid may have contributed to the failure to open. The braid can become damaged due to resheathing more than 2 times, over-manipulation, deployment technique, delivering/retracting delivery wire against resistance, or deploying/resheathing the braid against resistance. However, the cause of the braid damage could not be determined.H6.Coding updated based on analysis findings.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
