Event or Problem Description
Information was received from multiple sources (manufacturer representative, user facility) regarding a patient having oblique later al interbody fusion.It was reported that the bullet driver is damaged on the end that gets malleted.Also, the inserter and the guide are not holding onto pivox plate properly.There was no patient involvement reported.There were no further complications reported regarding the event.
