Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary: the implant had ti be revised yesterday (b)(6) 2025 due to the below (please see surgeons explanation below).I have also attached the product details and a picture. From surgeon: right thr 18 months ago.No problems.9 month history of clunking/subluxation.6 day history of squeaking and x ray showed liner dissociation.Intraop liner spun inferiorly with head articulating with shell. The product was not returned to j&j medtech orthopaedics; however, photos were provided for review. The photo investigation revealed that the delta cer head 12/14 32mm +5 appears to have signs of material transfer, most likely caused by the head being in contact with the acetabular cup after the disassociation event occurred. However, material transfer is not indicative of a device non-conformance.The observed disassociation could have contributed to the hip joint noise. As the device was not returned, an as-received condition could not be assessed, and a dimensional inspection and document/specification review were not completed. The review of the provided evidence confirmed the reported implant noise event involving the delta cer head 12/14 32mm +5.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.
