Device evaluation completed on january 06, 2026: the product was returned to mentor for evaluation.Mentor conducted a visual inspection, leak test and microscopic examination of the returned device.Visual analysis of the returned device revealed that the breast implant was found to have two (2) tears on the posterior view, measuring approximately 0.9 cm tear (a) and 0.1 cm tear (b).In addition, the tear (b) was noticed within a crease/fold.Leak testing was performed, in accordance with mentor procedures, and no additional leak sites were detected during the analysis.Microscopic examination was performed on the edges of the tears, and the cause of the tear (a) could not be identified.Additionally, instrument damage was not found on the edges of the tear (b).Mentor also performed a manufacturing record evaluation related to the reported complaint for the finished device lot number. No manufacturing non-conformances were identified as part of this evaluation.Although no conclusion could be reach on the cause of the tear (a), the instructions for use contain the following caution: causes of deflation of saline implants include but are not limited to the following events: crease fold failure, intraoperative or postoperative trauma, excessive stresses or manipulations as may occur during normal, daily routines including customary and purposeful trauma such as that which can occur during vigorous exercise, athletics, etc.The evaluation determined that the possible cause of the tear (b) is consistent with normal wear.Deflation can occur at any time after implantation, but it is more likely to occur the longer the implant is implanted.Creating wrinkles or folds should be avoided during implantation or other procedures as it can result in deflation at a later time.Breast implants may also simply wear out over time.Regarding the reported capsular contracture condition, there are not enough details to determine the factors that may have caused this condition.Capsular contracture in the patient¿s breast might be the result of the body¿s individual physiological response to the implantation of a foreign object in soft tissue.Further, capsular contracture is a known complication associated with these devices and is referenced in our current product insert data sheet. As part of mentor's quality process, all devices are manufactured, inspected, and released according to approved specifications.Based on the results of this investigation, it was determined that the product met the manufacturing release criteria, and that normal wear may have caused the observed defect. No corrective and preventive action (capa) is required now.Manufacturer¿s reference number: (b)(4).
