Event or Problem Description
It was reported that two leads were opened in the operating room and found to have bends at the distal end when the stylet was slightly pulled back and the lead was spun on the measuring tool. The surgeon decided not to implant these leads due to the observed bends. Two additional leads were opened and implanted, although the surgeon noted that all four leads appeared to have the same issue when the stylet was pulled back and the lead was twisted to check for a wobble or bend. However, all four leads appeared straight when the stylet was fully placed in the lead. The issue was resolved by implanting the additional leads, and no further information is available from the healthcare professional.
