It was reported that there was a revision total hip replacement. The corail stem has fractured at the neck region.Stem originally implanted on (b)(6) 2017.According to the surgeon the patient did not have a fall, was simply stretching to put his pants on and felt it give way.Surgeon also stated that the patients posterior hip capsule was deficient. The patient had previously complained of groin pain and had seen 2 other surgeons in relation to this for hip arthroscopy¿s where they performed psoas tendon releases.Stem was very well fixed and took considerable effort to remove from the femoral canal.Patients original surgery was for a hemiarthroplasty bipolar for a fractured neck of femur. This was later revised to a total hip with the femoral stem left insitu and the acetabular converted from a bipolar to a pinnacle thr.

Product complaint # (b)(4).This report is being submitted pursuant to the provisions of 21 cfr, part 803 (and/or part 4, as applicable). This report may be based on information which has not been investigated or verified prior to the required reporting date. This report does not reflect a conclusion by depuy synthes, or its employees that the report constitutes an admission that the product, depuy synthes, or its employees caused or contributed to the potential event described in this report. If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.H11 additional manufacturer narrative: h3, h6: the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed. The product was not returned to j&j medtech orthopaedics; however photos were provided for review. The x-ray and photo investigation revealed that the corail amt collar size 11 has fractured at the neck section.Additionally, the femoral stem presents sings of organic material and what appears to be bone ingrowth adhered to the fixation surface. With the information provided is not possible to determine a potential cause at this moment. The mode of failure of the device is multi-factorial and consideration must be given to all other potential influences such as surgical process, patient variables (e.G.Activity level and use), anatomical considerations and patient changes over time.Any conclusions from the investigational input provided have to be placed into context with all other relevant factors. It is highly recommended to the patient to consult with their healthcare professional for further assessment. As the device was not returned, an as-received condition could not be assessed, and a dimensional inspection and document/specification review were not completed. The overall complaint was confirmed as the observed condition of the corail amt collar size 11 would have contributed to the complained issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.

Product complaint # (b)(4).Investigation summary: revision total hip replacement - corail stem has fractured at the neck region.Stem originally implanted on (b)(6) 2017.According to the surgeon the patient did not have a fall, was simply stretching to put his pants on and felt it give way.Surgeon also stated that the patients posterior hip capsule was deficient. This was later revised to a total hip with the femoral stem left insitu and the acetabular converted from a bipolar to a pinnacle thr.The product was returned to depuy synthes for evaluation.Visual analysis of the returned device found that the corail amt collar size 11 presents sings of organic material and what appears to be bone ingrowth adhered to the fixation surface.Additionally the device was fractured from the neck section, the fractured fragment was returned assemble with the ceramic head. The majority of the fracture surface exhibited fatigue characteristics, which is consistent with clearly a low stress high cycle fatigue. There is no indication that a material or manufacturing issue has caused or contributed to the reported event. However, a definite root cause cannot be established due to there being multiple factors that may influence implant failure.Consideration must be given to all potential influences including but not limited to surgical process, patient variables (e.G.Activity level and use), anatomical considerations and patient changes over time.Any conclusions from the investigational input provided have to be placed into context with all other relevant factors.A manufacturing record evaluation was performed for the finished device product description: corail amt collar size 11 product code: 3l92501 lot number: 5282532 and no non-conformances or manufacturing irregularities were identified.A functional test was unable to be performed due to the post-manufacturing damage.A dimensional inspection was not performed since it was not applicable to the complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a manufacturing record evaluation was performed for the finished device product description: corail amt collar size 11 product code: 3l92501 lot number: 5282532 and no non-conformances or manufacturing irregularities were identified.Device history review: a manufacturing record evaluation was performed for the finished device product description: corail amt collar size 11 product code: 3l92501 lot number: 5282532 and no non-conformances or manufacturing irregularities were identified.H11 additional narrative: corrected: d4 (exp.Date & primary udi number), h4.
