Product complaint # (b)(4).Investigation summary: the broach can not be fastened to the broach handle where / when did the event occur? intra-op.What action was taken/required to manage the problem during the procedure? with same like product. Was a patient involved? no.Is litigation indicated or pending? no.The product was returned to j&j medtech orthopaedics for evaluation.Visual inspection revealed the device with signs of usage and scratches on the overall surface of the broach corail amt 11's post.Additionally, device post was found deformed at its middle section. The observed conditions of the device are consistent with a component failure that was caused by exposure to unintended forces like assembling the device in a misaligned position, or by applying excessive force while trialing that overload the device material.Properly handling and attention to the approved use of the device diminishes the risk of failure.A dimensional inspection was not performed since it was not applicable to the complaint condition.A functional test was unable to be performed due to the post-manufacturing damage. However the assembly issues can be confirmed due the observed damage on the post. The overall complaint was confirmed as the observed condition of the broach corail amt 11 would have contributed to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.Corrected: h3.
