A review of the provided image was performed by an intuitive surgical, inc.(isi) failure analysis engineer (fae). The following additional information was provided: the fae reported that it was hard to tell from the angle, but reported that there might be a grip cable break. No conclusive statement was provided.Intuitive surgical, inc.(isi) did receive the 8mm fenestrated bipolar forceps instrument to perform failure analysis.Failure analysis investigations did not confirm the customer reported complaint. The instrument was analyzed and it was found to be fully functional. There were no damaged cables.Additional findings: the instrument was found to have one bent grip, causing misalignment of the grips. There was a.260¿ offset at the tips. The grips did not show cracking damage. The probable root cause of bent grips is attributed to damage during use, as moderate misalignment of grip tips can be due to grasping either hard tissue/objects or collisions with other instruments.
