Event or Problem Description
On (b)(6) 2025, this patient underwent endovascular treatment for an abdominal aortic aneurysm and was implanted with gore® excluder® conformable aaa endoprosthesis with active control system (cxt). The cxt was advanced to the intended location and primary deployment was performed. The physician chose to move the device a little more proximal so the device was constrained and reposition proximally.Once it was positioned where he wanted we proceeded to 2nd stage deployment.Stage 2 was attempted and the red tab was moved back, but the clear handle just spun and wouldn't release. After applying more pressure the handle was able to be separated and the device deployed to 100 percent. The patient tolerated the procedure with good results.
